If you are unable to complete your forms before your visit, please arrive 15 minutes prior to your appointment time so that your visit will not be delayed. Also, bring any insurance booklets or forms or cards that you may have so we may assist you in filing your insurance claims.

In the interest of conserving your time, we will try to complete all treatment in as few visits as possible. Except for the emergency treatment of another patient, we strive to be prompt and respectful of your time.
